Frontline Healthcare Workers Face Similar Psychological Impact as War Veterans

For some, COVID-19 will provide years of trauma and moral injury

ALEXANDRIA, VIRGINIA, USA, August 31, 2020 / — Frontline healthcare workers, social service workers and other essential workers are being forced to make potential life and death decisions for themselves, their coworkers and patients and it’s likely to take a large toll on their mental health in years to come. Moral injury is an affliction of conscience that was first identified in combat veterans. Not to be mistaken for post-traumatic stress disorder, moral injury has its own treatment protocols. It happens under high stake situations where no good choice is possible or when emergency conditions require rapid responses by instinct or training without time to weigh a decision.

Rita Nakashima Brock, Ph.D., a noted researcher, author and speaker on moral injury, is senior vice president, moral injury at Volunteers of America. She leads the organization’s efforts to deepen understanding about moral injury in the many populations who experience it. Volunteers of America, one of the largest providers of housing and services for America’s most vulnerable, assists more than 1.5 million people annually. The organization has a large employment force of frontline care givers including those who care for more than 8,000 seniors housed in more than 20 locations. These are seniors in need of skilled nursing care and assisted living.

“The fight against the coronavirus is strikingly similar to battlefield medicine,” Brock explained. “The virus requires desperate and unrelenting encounters with patients, a lethal environment of high personal risk, an unseen lethal enemy, extreme physical and mental fatigue and inadequate resources—lack of PPE.”

Brock continued, “Increasingly swamped hospitals have forced frontline workers to decide who should live or die or they are forced to enact ‘do not resuscitate’ orders. These policies do not mitigate the moral impact on those who, after fighting to save people, must kill them because of an algorithm.”

For those on the front lines of Volunteers of America, there are equally dire decisions that include when and how to isolate those infected when caregivers must go from patient to patient. Since the onset of the virus, there have been questions as to how to get test kits and how to deal with cases when there is no test. Health departments are often overwhelmed, and caregivers are forced to make life-or-death decisions. They must also weigh the chances of contracting coronavirus and bringing it home to their families.

“Working past exhaustion only to see so many deaths, can have a moral impact,” said Brock. “And losing patients when you may have been the unknowing carrier, can be devastating.”

As with soldiers, healthcare and essential workers can be made to feel abandoned, unsupported and without leadership. Being sent into a potentially lethal situation without support can leave individuals feeling helpless and betrayed.

Betrayal wrecks trust, profoundly disrupts identity and destroys relationships. It is also suspected of causing or aggravating post-traumatic stress symptoms—nightmares, intrusive memories, hypervigilance, depression and irrational anger.

For those who cannot rectify their experience with their moral values, their conscience declares internal war. The aftermath of devastating experiences can accumulate relentlessly for weeks or months and lead to extreme pain and anguish. Often those experiencing moral injury turn to overworking, drinking or using drugs, self-isolation and silent despair, depression, abandonment of their career and, in some cases, suicide.

“It’s those individuals who are often the most experienced and humane professionals who have moral injury,” Brock explained. “They become crushed by the decisions they had to make and often find themselves in unrelenting grief, consumed by fury and humiliation at the authorities who failed them or bereft of faith or meaning.”

Brock recommends that we all need to support those on the frontlines by taking the time to regularly thank them. Being compassionate and sharing grief, even though it has to be virtual right now, is beneficial to those who are struggling. For those essential workers who continue to go to work in spite of their fear, it’s particularly important that people show their appreciation—thank the healthcare worker, the grocery store clerk, the social worker, the caregivers, restaurant workers, etc. Many of them are putting their lives on the line for little compensation as well.

“No one will come out of this pandemic unchanged,” Brock added. “But how we are changed is something we can influence.”

Two of Volunteers of America’s frontline caregivers, Kelly Tripp and Ronda Jones, carefully monitor and assist their co-workers in understanding the importance of seeking help if they need it. Tripp is executive director of the Volunteers of America Sojourn at Seneca Senior Behavioral Health Hospital in Tiffin, Ohio. Ronda Jones is director of nursing at Volunteers of America’s Laurel Manor Care Facility in Colorado Springs, Colorado. Both locations have experienced patients with COVID-19 and deaths. Both worry about the ongoing physical and mental wellbeing of their co-workers. “We’re constantly being forced to make quick decisions that could put others in danger,” Tripp explains. She offers the example of a wife who was in the hospital parking lot as her husband was being discharged from the hospital to a nursing home. The wife didn’t know if this might be the last time she could see and touch her husband. She wanted to kiss him. Tripp continued, “Even this small decision as to whether we follow the rules or show compassion and allow her to possibly kiss her loved one goodbye, depending on the near future outcome, could become a major haunting dilemma—either way. If one of them dies from this, it may forever weigh on our conscience.”

Jones has also had to make major decisions. Most of hers are regarding staffing. As cases rose at a senior center she was overseeing, workers became afraid to go to work. “They needed me to tell them it was safe and I couldn’t do that,” Jones explained. “Every day became a life or death situation and it will all remain a nightmare for me for the rest of my life.”

„Cu cooperarea orașului Daegu, Centrul de Atletism Daegu intenționează să asigure spațiul, iar Crucea Roșie intenționează să furnizeze echipamentul și personalul necesar în perioada 27 august – 4 septembrie. Ne exprimăm recunoștința față de oraș pentru că a oferit o locație pentru donațiile grupului. De asemenea, ne exprimăm recunoștința față de membrii congregației din organizația religioasă, Shincheonji, pentru a fi mai concret ”, a declarat domnul Kwon Jun-wook, director adjunct al KCDC.

Cercetarea și dezvoltarea tratamentului cu plasmă convalescentă sunt în curs de desfășurare la Institutul Național de Sănătate din cadrul Ministerului Sănătății și Bunăstării, în cooperare cu Green Cross (GC) Pharma, o companie de biotehnologie din Coreea de Sud.

În aceeași zi, Administrația SUA pentru Alimentație și Medicamente (FDA) a autorizat utilizarea de urgență a plasmei convalescente pentru tratamentul COVID-19. Experții spun că trebuie colectate mai multe date pentru a vedea beneficiile unui astfel de tratament.

Provocările majore ale cercetării eficacității și dezvoltării tratamentului cu plasmă convalescentă provin din furnizarea limitată a plasmei de la donatori, care trebuie să fie vindecați de COVID-19.

REPUBLIC OF KOREA , August 31, 2020 / — În timp ce numărul infectaților cu Covid-19 continuă să crească în întreaga lume, vocile comunității internaționale se ridică pentru a opri opresiunea religioasă care are loc în Coreea de Sud, cunoscut ca o țară exemplară referitor la carantina de Covid-19.

Pe 17 august coaliția liderilor caraibeni pentru pace (CCLP), formată din foștii și actualii lideri din Caraibe, inclusiv fostul președinte al Consiliului pentru Drepturile Omului al Națiunilor Unite (UNHRC), a trimis o scrisoare comună ministrului sud-coreean de externe Kang Kyung-wha și înaltul Comisar al Națiunilor Unite pentru Drepturile Omului, Michelle Bachelet.

Aceștia au spus în scrisoare că guvernele, chiar și ca răspuns la urgența pandemiei, trebuie să-și asume responsabilitatea pentru protecția drepturilor omului indiferent de religie, rasă, etnie sau statut socioeconomic și și-au exprimat îngrijorarea pentru opresiunea continuă împotriva Bisericii Shincheonji, Denominație creștină din Coreea de Sud care a suferit de o infecție neașteptată în masă la începutul acestui an.

Înainte de scrisoarea comună, 11 ONG-uri, inclusiv Coordonarea Europeană a Asociațiilor și Persoanelor pentru Libertatea Conștiinței (CAP-LC), au transmis Secretarului General al ONU „raportul anual pentru înaltul comisar al ONU pentru Drepturile Omului” cu privire la discriminarea țintită în mod necorespunzător împotriva Bisericii Shincheonji.

Raportul anual a fost intitulat „Membrii Shincheonji – Țapul Ispășitor pentru COVID-19 în Republica Coreea”.

Scrisoarea a subliniat succint faptele din referitoare la Shincheonji și Covid-19 după cum urmează;
〮 Covid-19 a fost introdus în Coreea de Sud din China.
〮 Conform Centrelor Coreene pentru Controlul și Prevenirea Bolilor, virusul era deja prevalent în orașul Daegu înainte de confirmarea (Shincheonji) Pacientului 31 (din Daegu).
〮 Refuzul guvernului de a închide frontiera în China a contribuit în mare măsură la izbucnirea virusului.
〮 Față de nemulțumirea publică din ce în ce mai mare cu privire la faptul că guvernul nu a impus o restricție de călătorie Chinei, ministrul Justiției, Choo Mi-ae, a ordonat urmărirea penală și investigarea Shincheonji.
〮 Vice-ministrul Sănătății a confirmat că lista informațiilor de identificare privată colectate nu este cu mult diferită de cea colectată și verificată de guvern.
〮 Procurorii au arestat oficialii din Shincheonji pe motiv că lista membrilor congregației depuse de Shincheonji nu era completă.

Referindu-se la raportul „Fișă informativă asupra răspunsului global la coronavirus (COVID-19) și la impactul asupra practicilor religioase și a libertății religioase” de către Comisia Statelor Unite privind libertatea religioasă internațională (USCIRF), scrisoarea comună a reiterat faptul că Coreea de Sud furnizează un exemplu viu al modului în care urgențele de sănătate publică pot crește riscul pentru grupurile religioase marginalizate.

Shincheonji Church Receives Praise for Becoming the Leader in South Korea’s Plasma Donations

NEW YORK, NEW YORK, USA, August 30, 2020 / — On August 27, COVID-19 recoveries of the Daegu Branch of Shincheonji, Church of Jesus, The Temple of the Tabernacle of Testimony (SCJ), participated in a plasma donation held at the Daegu Athletics Promotion Center. A total of 1,100 people will participate in the second plasma donation, which runs until September 4. A total of 628 people have completed plasma donations thus far. At the completion of this donation period 1,700 SCJ congregants will have participated in plasma donations. The development of a treatment system in Korea is becoming more tangible with the group’s plasma donation. In February, when the number of confirmed cases of Shincheonji soared, Chairman Lee Man-hee of Shincheonji urged believers to pray to God for the cure.

Recently, critically ill patients are rapidly increasing due to the re-proliferation of the coronavirus. There have been over 300 confirmed patients. Therefore, treatment is urgently needed more than ever before.

The South Korea’s Central Disaster and Safety Countermeasures Headquarters (CDSCH) thanked Shincheonji for the collective blood plasma donation. Kwon Joon-wook, deputy director of CDSCH expressed his gratitude at two press briefings this week. "[The] group plasma donation for the development of treatments were made by the members of Shincheonji who were infected between February and March." He explained if a patient had completely recovered by April and May, then August and September is the optimal time for them to donate plasma. This is because coronary antibodies produced in plasma decrease after four months of full recovery.
Recovering plasma of fully cured patients is drawing attention because there is a hypothesis that a "neutral antibody" in the plasma can potentially suppress the growth of the virus. In addition, if large amounts of immunoglobulin is extracted from the plasma of fully recovered patients, developing a cure is more feasible. The blood treatment, which is being developed by Green Cross in Korea, has already been approved by the Ministry of Food and Drug Safety for Phase 2 clinical trials.

Securing large amounts of COVID-19 plasma from fully recovered patients is not easy, even in foreign countries, where there were far more confirmed cases than in Korea. Many patients have suffered for long and are tired of elongated treatment, and therefore are reluctant to donate their plasma.

Currently, Shincheonji, which suffered a massive collective infection in February and March, has become the leader in Korea’s blood plasma donations.
